because of the increased cases in which insurance companies in liquidation after the notification of the damage from liability insurance of a motor vehicle when your car is damaged and instance company  refuses to pay indemnity even if you  completed the Accident Report, it is appropriate to draw the guilty person -the driver this Statutory declaration of the driver who is responsibility for collisions and damage.  See attached form to fill, especially in cases:

- The guilty driver is different from the owner of the vehicle.

- Report of the accident is not fully clear fault of guilty driver.

- It is suspected that the guilty person has not informed this incident to his insurer.
